1. Background

Although during the last ten years there is progress in the prevention of nutrition problems in Indonesia, but when compared to some of the Asean countries like Thailand, the prevalence of malnutrition is particularly severe malnutrition and malnutrition in Indonesia is still high. One of the efforts to accelerate decline in prevalence of nutrition problems can be done with great attention to data growth and nutritional status of children who later became the basis for preparing nutrition program (Sukiman, 2002).

Recording and reporting system of integrated health centers (SP2TP) can potentially play a role in supporting many health management processes. However, various SP2TP data available to support health management can not be used optimally because of various matters relating to the design of the system, besides that enjoy unlimited capacity of resources in health centers in terms of both humans and support facilities may not optimally utilize the data and SP2TP other information supporting management in health centers (MOH, 1997).

According to research, Nina (2003), it is known that the existing data for program planning is not complete, it is also known that the quality of health nutrition program data is invalid and incomplete. According to some of the factors causing them are functional personnel / paramedics, so the recording and reporting less attention than the recording and reporting system there is still need simplification and improvement of another.

Based on observations during this writer, the quality of data in health nutrition program Tandun still sketchy. In connection with the above description is necessary to study recording and reporting on the implementation of nutrition programs in health Tandun Rokan Hulu regency in 2010.
